We are seeking a Life Care Coordinator to assist with the implementation and execution of the Lifesprk Experience delivered in the campus setting including customer service and communication, office management, Life Care Management assistance, and technology assistance.

Qualifications/Requirements

  • College degree in healthcare or business field (preferred)
  • Experience in medical field including terminology
  • Minimum of one year of experience as an LPN
  • Minimum of one year of experience in admin/office management role
  • Proficient with technology and technological applications: Microsoft Office, Smart Phone, Calendaring, Wi-Fi, EMR, Internet Browsing, Printer

Life Care Management

  • Daily tracking, filing, printing and sorting
  • Packing slips (supplies and medications)
  • Flow sheets – identify gaps; ensure completion; file in client’s office chart
  • Daily assignment sheets
  • Assist with client record management
  • Filing, scanning, thinning
  • Entering/updating client info in EMR (electronic medical record)
  • Entering client info at start-of-care
  • Scan service agreement to accounting
  • Manage/coordinate long-term care insurance
  • Liaison to/from client and company PRN
  • Submit monthly invoices (Lifesprk & Silvercrest) or other requested materials from client or insurer
  • Communicate changes or concerns with Life Care Manager (LCM)
  • Assist LCMs with client tasks as requested/INR coordination
  • Medication set up
  • Physician order coordination
  • Client visits
