FAQ – Education Minnesota/ESI

Frequently Asked Questions
These are just a sampling of the great questions we’ve received from other Education Minnesota ESI members inquiring about Senior Care Navigation. If you have others, please reach out to us directly at 952-345-3317. And be sure to check out FAQs about Lifesprk

Q: What is free and what is paid?

Senior Care Navigation is a free service connecting you to whatever you need to optimize you or your loved one’s aging experience – help getting answers, finding resources and determining options. This service is available to you no matter where you live in Minnesota, Wisconsin or anywhere else in the country.

If the time comes to need additional services or coordination, we suggest a more in-depth Life Consultation (this portion is also free) to better understand your situation and identify all your options, which may include additional Lifesprk services. Fees for this are on a case-by-case basis because it’s based on your unique situation.

Q: When should I consider a free Life Consultation?

Anytime you are concerned about your loved one. But often some of those triggers might be a change in medication or diagnosis of a chronic illness, concerns with recent falls, consider a move to senior housing, recent hospitalization or repeat trips to the emergency room.

Q: What are some questions people have asked through Senior Care Navigation?

We get many kinds of questions – whatever is important to you. Some have included: can I find a hospital bed, what’s the difference between Medicare and Medicaid, etc. There is no limit to the number of calls – you can call Senior Care Navigation as many times as you want.

Q: What types of reliable resources can you connect to?

We connect people to anything – across all 7 elements of wellbeing – that they need to live a sparked life. Things like home repairs, financial advisors such as EFS Advisors, transportation, dog walkers, housing options, and much more. They are all vetted and trusted by us.

Q: What is a Life Care Management?

This proactive service offers the care and guidance that an increasing body of research shows to be the most important element in producing good results (e.g., helping you thrive at home or wherever you live). With Life Care Management, you will get matched with a dedicated Life Care Manager (nurse) who will provide a very high level of ongoing guidance with personalized attention.

  • Your Life Care Manager becomes your go-to resource for all aspects of living well, and can help you, your family, and your health care professionals coordinate and manage your care.

This service is the key factor in reducing your overall costs while improving your wellbeing and helping you to avoid future crises. About 33% of all our clients just have Life Care Management.

  • Most importantly, your Life Care Manager will focus on you as a person and help you live life on your terms – whether that means volunteering at the local library, birdwatching, golfing, or just spending time with family. At Lifesprk, we call that living a ‘sparked life’ and that’s our goal for all our clients.

Q: Do you offer caregiver services?

Yes absolutely, some of our clients also need caregiving help for activities of daily living as well as companionship and other needs. Our Life Care Managers work closely with our caregivers to ensure they provide a whole person approach and stay focused on your personal priorities and goals. Caregiving support goes beyond basic care to help you live a sparked life, and can include:

  • Homemakers, companions, and home aides for 4- 24 hours a day, including live-ins
  • Light housekeeping
  • Shopping and errands
  • Meal preparation
  • Walking/mobility
  • Bathing assistance
  • Dressing and grooming

You and your family can continue to use the free Senior Care Navigation service for connections to reliable resources and answers to common questions even while a loved one is a client. However, you can also access Senior Care Navigation through your loved one’s Life Care Manager.

Q: Do you offer Home Health?

In addition to Life Care Manager, we also offer home health services as another option for senior care. Because our team works closely together, when your Medicare home health benefit runs out (this tends to be more short-term in nature) we can easily maintain your continuum of care by bringing on board Lifesprk’s whole person approach for long-term needs, too.
